Severide is concealing a significant shoulder injury he suffered during the fire that took Andy Darden's life, and he struggles with feelings of guilt as he attempts to rescue a construction worker trapped in a building collapse. The situation worsens when Darden's widow, Heather, confronts Severide at the annual firefighters' barbecue, holding him responsible for her husband's death. The team receives a distress call from Hancock Tower, where two teenagers are trapped in a car under the wreckage of a fallen window washer's cart. Meanwhile, Casey and Hallie are trying to determine what lies ahead for their relationship.
