During a confrontation with a demon, Dean suffers an electrocution that causes irreversible harm to his heart, giving him just a few months to live. Heartbroken, Sam frantically seeks a solution to save his brother and comes across a preacher who promises to cure the incurable. However, the brothers soon uncover a dark secret: the preacher is receiving assistance from the Grim Reaper, who is swapping lives, deeming some more worthy than others. This revelation forces Sam and Dean to confront the moral implications of their desperate search for a miracle.
